Dr. Horn was a medical doctor from a line of presitgious doctors, among them his late father, and his identical twin.

When he announced to his father that he wanted to help the poor in the East End, his father felt it to be a dishonour to the family name, and disinherited his son, and barred him from the family home. However, the true reason was that his father was a high-ranking member of the Royal Medical Society, and felt that his son would never have been able to follow the society's morally questionable creed.

It was only thanks to the support of his brother he was able to pursue his dream, and opened a clinic and began to help those in the area around the Club Mary.

His youth and genteel manner were noted by his patients; he would provide medical care for the local prostitutes without any worries about any kind of social stigma from his peers - often checking up on them for any concerns they had.

He was also a part-time coroner, being called upon by the police to perform autopsies.

In a misunderstanding, he was initially called to perform his duties as a coroner by Inspector Victor Dickens, when The Asian Man abducted Pacey Jones from the Club Mary - although there was no body to examine.

He was later tasked with performing an autopsy on the burnt remains of the Asian Man, following his murder in a graveyard. At this point he became suspected by the locals of Club Mary to have some involvement in the abduction of Pacey.

He recieved a telegram telling him to come to Club Mary, where he came across a bloodied, frantic Pacey and took her inside - fortunately he she was unharmed, but this only raised suspiscion against him further when he revealed that she had a cesarean section against her will, but it was performed with surgical precison.

Koudelka Iasant read his palm to search for any indication that he was the one behind the kidnappings, but could find nothing. In fury, Joshua pounced on him, while he insisted that he was innocent. It was then after trying to shove Joshua off him that he discovered that Joshua was actually a girl.

Still unsure about the him, Kitty escorted him as he was running behind on his patients, however his brother disguised himself as him, and attempted to murder Koudelka, before failing and fleeing to a secret cellar.

Still walking with Kitty, he told her of his family history, before bumping into Koudelka, Joshua, and Inspector Dickens who had tracked down the other Dr Horn.

He was utterly horrified to discover what his brother had been doing; abducting pregnant women, and keeping their foetuses in preperation for a mass sacrifice. Even moreso when his brother told him that the only reason he had supported his work in the East End was to gain easy access to the medical records of pregnant women.

His brother attempted to murder them by unleashing mosquitoes to infect them with Black Fever, but ended up infected himself, thanks to the sudden appearance of Dr Hans. They fled the cellar, which exploded due to a bomb left by one of De Gaulle's moth familiars.

There was little time for Dr Horn to consider things however as Joshua collapsed from the early stages of Black Fever. When Dr Hans told him of a cure stored in the Royal Medical Society's HQ, he insisted on going, feeling responsible for his brother's actions, but was persuaded to stay.

Later, he and the residents of Club Mary are later taken to Nemeton Monastery as hostages.

In the epilogue, it was revealed that he went to America to continue his practice.
